  • Define the Situation: What is the Problem? What customer is affected? What requirements still need to be met? What is the Scope of the Problem? How often has it occurred? Define the measure of completion.


  • Quick Fix: Temporary solution when possible. Prevent the problem from reaching the customer—rework, replace, re-deliver, communicate plan to prevent surprises.


  • Identify Root Cause: Look for all possible causes, select the most likely cause, verify with data, check process flow and checklists, brainstorm.


  • Take Corrective Action: Assemble key people, select the best solution, plan and communicate it, and determine who will implement it. What is the resolution date? Document implementation.


  • Evaluate & Follow-up: Is the problem permanently eliminated? Is the process mistake-proof? Has the measure of completion been met? Have audits/reviews been set up to ensure permanence?
